And it operates differently than a typical government program. Insurance companies design and offer competing coverage plans in each state under Part D. Enrollees can choose the plan that best meets their medical and financial needs.

This level of choice and consumerism was intended to keep costs low while also providing seniors with options. And it was effective.

Despite initial claims that it would break the bank and bankrupt seniors, Medicare Part D turned out to be the rare government program that came in significantly under budget. Part D’s cost to taxpayers was predicted to exceed $122.88 billion in 2012. That year’s actual cost was only $55 billion.

Part D also provides Iowans with access to 30 prescription drug plans. Premiums in the state begin at $15.70.

Despite the variety of affordable options, there is a push in Washington to dismantle the program. Part D is too expensive, according to critics, and the government should intervene in price negotiations with drug manufacturers.

In November 2017, the Iowa State Association of Counties unanimously passed a resolution encouraging member counties to join in the litigation against pharmaceutical companies to hold them accountable for the country’s widespread opioid epidemic.

For downplaying the risks of opioids and aggressively marketing the drugs to treat chronic pain, 47 Iowa counties filed lawsuits against five pharmaceutical companies, their subsidiaries, and three physicians.

Story County is one of 53 unrepresented counties in the settlement, but it was asked to join to reach the maximum settlement value. A full settlement will result from municipalities’ full participation. The state will receive $143 million if all litigants, excluding Story County, participate.

Over the last two decades, the opioid epidemic has claimed the lives of over 500,000 Americans. According to Crueger Dickinson, a law firm representing Iowa counties, increased costs for counties have been linked to the crisis, including law enforcement, autopsies, and emergency medical services.

The MPCA, on the other hand, has determined that the established pharmaceutical reverse distribution system provides an environmentally safe method of dealing with waste pharmaceuticals. As a result, the MPCA allows pharmaceutical waste to be managed in Minnesota health care facilities through pharmaceutical reverse distribution. if they comply with the conditions in this fact sheet, instead of meeting the full hazardous waste requirements.

  • What are the conditions for pharmaceutical reverse distribution in Minnesota?

To manage unevaluated or hazardous pharmaceutical waste via reverse distribution, you must first:

  • If necessary, obtain a Hazardous Waste Identification Number (HWID, also known as an EPA ID#) from the MPCA.
  • Confirm that the initial reverse distributor who will receive your pharmaceuticals is licensed as a pharmaceutical manufacturer or wholesaler by the Minnesota Board of Pharmacy (Board). Before disposal, pharmaceuticals may be transferred through an unlimited number of reverse distributors.
  • Verify that the reverse distributor is also a DEA Registrant if the pharmaceuticals are controlled substances regulated by the Board or the United States Drug Enforcement Administration (DEA).
  • Accumulate and ship only pharmaceuticals in original manufacturer packaging or recalled pharmaceuticals in any condition for pharmaceutical reverse distribution. Aerosol-dispensed pharmaceuticals are manageable through reverse distribution, but not all reverse distributors accept them. Disinfectants and cleaners do not qualify as pharmaceuticals.
  • Comply with all of your pharmaceutical reverse distributor’s return policies (s).
  • If the pharmaceuticals to be shipped for reverse distribution are classified as hazardous materials under the Hazardous Materials Regulations (HMR) of the United States Department of Transportation (DOT), prepare a shipping paper by the HMR. For pharmaceutical shipments managed through reverse distribution, do not qualify or use a hazardous waste manifest. Maintain copies of shipping documents for three years. Note: When pharmaceutical products that were initially shipped to you as Limited Quantities, formerly known as ORM-D Consumer Commodities, become waste, they may still be eligible for Limited Quantity reduced requirements, including exemption from shipping papers, under the HMR. The ORM-D name will be phased out on December 31, 2020, but the Limited Quantity category will continue.
  • Obtain and keep a copy of the reverse distributor’s delivery confirmation for your shipments.
