The clinic was founded in 1921 by George F. Straub. Dr. Straub was born in Germany and began practicing medicine in Hawaii in 1907.  In 1910, he had the notion of organizing a group of physicians to serve his patients with better, more specialized treatment. However, it took more than a decade to bring this concept to existence.

The initial step was to erect a wood-frame building at 410 South Beretania Street in 1912. On the first floor, Straub constructed his office, and on the second floor, he established his house.

By 1916, his business had grown to the point that he hired Guy C. Milnor as an assistant. Obstetrics and Gynecology, Surgery, Internal Medicine, ENT, and Clinical Pathology were among the five key specialties that Straub envisioned for his clinic.
